SFQ information for Direct Service Providers
The Sustaining Facility Quality Program rewards facilities and individuals who meet certain criteria in their efforts to continue providing excellent, high-quality early childhood intervention.
Specific criteria for both individuals and facilities are outlined on the following pages:
Calendar of Due Dates:
Attendance due dates, deadlines to submit requests for start-up funds, and other reporting due dates will all be published in periodic mailings to the facilities and individuals participating in the programs.
Forms:
Reporting forms will also be sent in periodic mailings, with instructions about returning them to Smart Start.
